1. Background/Overview
During your work placement, you will be undertaking a variety of duties and tasks that relate to your course and the role of a community services worker or manager.
You will need to successfully demonstrate that you can perform tasks required in the work environment and collect evidence.
This portfolio will be broken down into tasks, and these need to be submitted in three parts following the training and assessment plan agreed upon with your Workplace Assessor. This is Portfolio 3 of 3.
2. Portfolio Information
You will be required to complete each task in this portfolio at your Host Organisation under the supervision of an approved Workplace Supervisor (See Workplace Assessment Guide).
Your Open Colleges Work Place Assessor will conduct assessments in the work place for key tasks in the Portfolio. These are identified by the use of the following text:
This Task must be assessed in the work place by your Colleges Work Place Assessor (WPA).
Date: WPA name: WPA signature: Outcome (S/NYS): Through discussions with your WPA, approximately half way through your work placement, a work place visit will be arranged to enable observation/demonstration of these key tasks. Take every opportunity to practice these tasks with your colleagues or supervisor in advance of your WPA visit. You may also need to prepare some Task documentation in readiness for your WPA visit – this will be clearly indicated in the relevant task instructions. Where required, your WPA may also ask a short contingency question based on the task.
Your WPA will deem your assessment performance as either Satisfactory (S) or Not Yet Satisfactory (NYS). Where your assessment performance is deemed Not Yet Satisfactory (NYS), you will be required to participate in the relevant assessment again.
Before you move onto the last part of the portfolio, make sure you have completed all the tasks in this section and uploaded your work to OpenSpace.

Role of your Colleges Workplace Assessor
Your Colleges Workplace Assessor will observe you in the workplace completing each task from your Structured Workplace Learning (SWL) portfolios of evidence. They will conduct this observation in order to assess whether you are competent for the relevant unit/s of competency.
De-identifying workplace documents
Any workplace documents that you use for your portfolio task submissions must be de-identified. That is, please ensure real names and addresses are removed from any forms before you submit them as part of your assessment documentation.
Audio and video recordings
Some of the tasks in the portfolios will require role-play activities that will need to be recorded as an audio file or a video file. Depending on the instructions for the relevant task, you may need to ask a friend, colleague or study buddy to play the part of the client whilst you play the part of the case worker or manager. For each audio or video recording, the following instructions must be followed:

This Task must be assessed in the work place by your Colleges Work Place Assessor (WPA).
Date: WPA name: WPA signature: Outcome (S/NYS): You will be required to perform this Task in the presence of your Colleges Work Place Assessor during their workplace site visit. At the completion of their observation during their site visit you will be provided with feedback in regards to your performance.
In addition, your Work Place Assessor may liaise with your Workplace Supervisor to gather any supplementary evidence to support assessment results.
Instructions
Approximately half way through you work placement, during a work placement visit, undergo a formal supervision session with your Colleges Work Place Assessor aimed to review the effectiveness of your practice in the workplace. During this supervision you should:

reflect on your own values and attitudes
review the effectiveness of work and/or services provided to clients
identify how you have revised work and/or services to enhance the client outcomes and better address their social and cultural issues
display a willingness to learn from the feedback given.
Complete either a document from your workplace or use the Supervision Record template (Version 1) provided in the Appendix 8 that provides an overview of the supervision session.

Task 14A: Facilitate workplace debriefing This is Task 14A of 16
The purpose of workplace debriefing and supervision of staff is to provide a safe and confidential environment for staff to discuss and reflect on both personal and professional responses in the workplace.
Task
Conduct and submit two (2) written supervision records with two (2) different staff members. The focus of the sessions is self-care including addressing and monitoring strategies to manage stress levels and emotional wellbeing.
If you do not have the opportunity to conduct supervision sessions in the workplace, ask friends, family members or study buddies to play the role of a staff members.
You can use a document from your workplace or use the Supervision Record template provided in Appendix 9 and 10.

Instructions – Role play with WPA
Workplace debriefing is an especially important tool to assist staff review distressing or emergency situations at work. In order to successfully facilitate this debriefing, it is essential to provide a safe and confidential environment for staff to discuss and reflect on both personal and professional responses in the workplace.
This task is a role play with your Colleges Work Place Assessor. You will play the part of the case manager and your WPA will play the part of the case worker in your team who has experienced an incident.
You might like to use a real incident that you have witnessed occurring in the workplace during your placement, or, if you don’t have a suitable incident to review, you can use the suggested scenario (provided on the next page) on which to base your role play. At the beginning of the role play, you should explain if you will be debriefing a real incident that you have witnessed, or if you will be using the suggested scenario we have provided.
The role play should be a maximum of 5 minutes long and demonstrate that you are able to:
follow standard procedures and protocols
create a safe environment
facilitate discussion
use appropriate debriefing techniques including
best practice interventions
crisis intervention
structured debriefing
internal and external referral sources
use appropriate questioning.
Suggested scenario for Task 14B

Conduct the role play per the instructions in the presence of the WPA.
You work at CareShore in the community services office. You are responsible for undertaking case meetings to plan, monitor and review service provision for clients. Part of your role also involves obligations toward other employees, including debriefing with employees after an incident.
Last night, a CareShore worker, Jo, was working in the office. A man entered the office around 9pm saying that he felt depressed and wanted to talk to someone. Jo spoke with the man. She asked him for his name. He refused to give it. Jo then asked if he would like to come in and have a chat.
The man didn’t respond to Jo’s questions. He started pacing the floor and was becoming increasingly agitated. Then, without saying anything, the man started punching and kicking a wall. He caused quite a lot of damage.
Another colleague heard the commotion from his office and quickly called 000. While waiting for assistance, Jo moved to the back office and left the man in the foyer. The police soon arrived and arrested the man.

This task requires you to demonstrate your ability to undertake a leadership role in the delivery of quality case management. This includes sharing information and providing advice on practice issues relating to case management within the organisation.
Part 1 – complete before WPA visit
Develop and submit a generic supervision agenda you can follow for staff who provide case management services. Ensure the agenda allows for discussion on:
Client overview
Client support strategies
Best practice principles
Monitoring and evaluation of client progress
Guidance and recommendations
File upload instructions
Save your file on your computer using the naming convention: T15_studentnumber_P1.docx. Example: T1C_12345568_P1.docx. The File should be uploaded to the Portfolio section in Open Space when the Portfolio has been completed
Part 2 – to be conducted with WPA
Using the agenda as a framework, role play you providing formal supervision to two (2) case workers, with your Colleges WPA playing the case worker roles. The two separate supervision sessions should be a maximum of 10 minutes long and demonstrate that you are able to lead and advise case workers in relation to the provision of case management services.
You may wish to refer back to Portfolio 2 Task 6 and select one (1) of the complex clients you completed an interview/assessment or referencing meeting with. Use these clients as the basis for the supervision of the case workers, however, you will be required to provide the written report that provides an overview of that client to your WPA prior to role play.

Task 16: Reflective practice This is Task 16 of 16
Part A
Effective learning allows opportunities for you as the student to critically reflect on your practical experiences. It supports understanding and allows you to make meaningful connections between course theory and putting that into practice through the development of skills.
Select three (3) learning experiences that have occurred for you during your work placement and create a reflective journal entry for each.
Using Kolb’s Learning Cycle structure your reflections to include:
An overview of the learning experience
Your observations and reflections after you have reviewed the experience
What you learnt from the experience

A Personal Development Plan (PDP) is a valuable tool to help you organise and plan for your professional development. Its main aim is to help you achieve both short and long-term career objectives.

And/or
The student has not obtained their supervisor’s signature on the report.
And /or
The Workplace Supervisor has not confirmed in a checkpoint with the student’s workplace assessor that they did indeed sign the report.
Task 14B
Facilitate workplace debriefing The student has successfully conducted a role play of not longer than 5 minutes (playing the part of a case manager) with the WPA playing the part of a case worker in their team who has experienced and incident, either using a real incident that they have witnessed in the workplace or using the suggested scenario provided on Page 6 of this document.
And
The role play accurately follows the task instructions and demonstrate that the student is able to:
follow standard procedures and protocols
create a safe environment
facilitate discussion
use appropriate debriefing techniques including:
best practice interventions
crisis intervention
structured debriefing
internal and external referral sources
use appropriate questioning. The student has not successfully conducted a role play of not longer than 5 minutes (playing the part of a case manager) with the WPA playing the part of a case worker in their team who has experienced and incident, either using a real incident that they have witnessed in the workplace or using the suggested scenario provided on Page 6 of this document.
