For this task you are required:

1. to evaluate and prioritise the needs of any three (3) clients based on the information collected in your workplace and develop a person-centred individualised plan to meet the aspirations, needs, rights and preferences of the clients. You are to develop an individualised plan for three (3) clients with complex support issues at your workplace depending upon the identified needs of the client using the given template. You must provide at least three (3) identified needs of each client in the given template.

An individualised plan must provide information to address the following criteria.
