Analyse and respond to client health information
This unit of competency describes the skills and knowledge required to analyse client health information and then to plan appropriate health services within the scope of own role. It requires an in-depth knowledge of anatomy and physiology.
This unit applies to individuals who plan and provide services with some level of autonomy. Some disciplines will require a state/territory regulatory determination regarding delegation and supervision.
Perform clinical assessments and contribute to planning nursing care
This unit of competency describes the skills and knowledge required, within the scope of practice, to perform preliminary and ongoing physical health assessments of all body systems, gathering data that contributes to a person’s individualised health care plan.
This unit applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia regulatory authority legislative requirements.
Implement and monitor care for a person with chronic health problems
This unit describes the skills and knowledge required to contribute to the care of a person with chronic health problems by performing nursing interventions that support the person’s needs and assist them in maintaining an optimal lifestyle.
This unit applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia regulatory authority legislative requirements.
Assessment Requirements
For more information relating to the RTO assessment requirements, including the management of student progression, please refer to the NSW Health Student Handbook. For course specific assessment requirements, please refer to the Course Guide.
Students are required to complete all the assessment tasks included in this document. The assessment tasks are summarised in the Assessment Plan in this document.
Your assessor will discuss all of the assessment requirements with you prior to undertaking this cluster. The deadline for completion of this assessment will also be discussed at this time.
You can hand in your completed assessments before it is due. If however, you cannot hand it in by the due date, you will need to negotiate with your Program Coordinator prior to the assessment due date (except in circumstances that prevent you from doing so).
Should you fail to submit your assessment on time and without prior approval for an extension, after a period of two weeks you will be deemed not competent and your manager will be notified of this outcome. If at a later date you are able to negotiate the resubmission of your assessment with your Program Coordinator and you are deemed competent, this outcome will be changed.
Following submission of your assessment, you will be provided with feedback and if required, further training may be arranged to facilitate achievement of competency.
If on second attempt you are unable to meet the requirements, an interview will be held with your Program Coordinator to assist further development and achievement of competency.
The Student has the right to appeal an assessment decision at any time. (Please refer to NSW RTO Student Handbook for the Appeals process).
Your assessor will advise you of the due dates for each of the assessments, or in the case of a workplace observation/s, the date/s and time/s of the observation.
Assessment Task 1 – Case Study 1
The case study will assess your knowledge and understanding of anatomy and physiology and how you apply it to related altered states. You will be required to analyse health information, demonstrate critical thinking and problem solving skills, identify health issues and plan appropriate delivery of patient care.

This case study will assess your knowledge and understanding of pathophysiology and chronic disease. You will be required to analyse health information, demonstrate critical thinking and problem solving skills, and plan appropriate delivery of patient care.

Assessment Task 4 – Workplace Project
During your ward allocation in Professional Experience Placement (PEP) 2, you are required to provide care for one (1) patient with a chronic health condition. This patient will become the case study for your workplace project in which you need to complete holistic person-centred care from admission through to discharge. This assessment task requires you to present your workplace project to your group during PEP 2.
Assessment Task 5 – Health Promotion
During your ward allocation in Professional Experience Placement (PEP) 2 you are required to provide care for two (2) patients with Diabetes Mellitus. You are required to develop and design a health promotion goal for each individual patient. You are to discuss the health promotion goals with your patients and their families.
Assessment Task 6 – Simulated Skills Assessments
During Learning Unit 2 you are given access to the simulation laboratories to participate in clinical experience of taking vital signs, neurological observations, neurovascular observations, blood glucose monitoring, physical assessment, admission/ discharge, and reestablishment of oxygen therapy.
