The purpose of this assessment is to examine your ability to provide holistic care to a client affected by an acute or chronic wound, and to demonstrate the integration of the theory provided throughout this unit, into the context of your clinical practice.

• Select a client from your clinical setting, who presents with an acute and/or chronic wound.
• Describe the client’s clinical presentation and significant history
• Summarise the results of your holistic assessment, including the extent to which factors such as, the client’s; ethnicity, culture, gender, age, disability, economic and social status, may have impacted on their general condition and presenting wound
• Identify the nature of the wound/s (e.g. acute, chronic and/or infected)
• Utilise the theory provided throughout this subject to develop an evidence based management plan for this client.
• Appraise the consultation and referral pathways utilised by the nurse- as a member of the multi-disciplinary team- to optimise outcomes and facilitate person centred care
• Support your discussion with reference to current literature in order to demonstrate clinical reasoning and evidence based practice
